An altered dominant piles the sharpened and flattened extensions onto a dominant seventh, so Ealt is maximum tension looking for a resolution. It's core jazz vocabulary, typically the V chord immediately before the tonic, and pairs naturally with the altered scale. Not a chord for gentle contexts — its job is to make the resolution feel earned.
